How is this different from a normal site build?

The order. The page plan, headings and internal links are decided from search data before design starts, rather than being adjusted after the design is signed off.

Does it cost more?

The build is priced the same. The keyword research adds a stage at the start, and it saves a separate optimisation project later.

Will it still look good?

Yes. Search constraints affect structure, headings and speed — not whether the design has any character. The prototypes will show you.
